Malignant Tumors of the Lung , An Analysis of 84 Cases

Abstract

|
During the period of six and a half years from January, 1957 to June, 1963, 84 cases of malignant tumor of lung were seen at Severance Hospital, Yonsei University Medical Center.
Of these 84 cases, the tumor was proved histologically in 70 cases, and in the remainine 14 cases, the diagnosis of the tumor was made based on the clinical findings. The youngest patient was 29 years old, and the oldest one was 75 years of age. The males were affected five times as much as the females. The average duration of symptoms were Ⅱ months. The predominant clinical findings were as follows; presence of tumor on X-ray picture without disabling symptoms in 39%. atelectasis and／or pneumonitis in 25%, cathexia in 12%, pleural effusion with atelectasis in 10%, lung abscess in 10%, superior vena cava obstruction in 2%, and massive effusion and empyema with atelectasis in 1%.
Bronchoscopy was done in 71 cases, and, of these, tumor was visualized and biopsited in 35 cases or 49%. In 16 cases or 23%, there was indirect evidence of tumor bronchoscopically, such as fixed or widened carina, narrowing of the bronchial lumen by external compression. Bronchoscopy was negative in 20 cases or 28%. Bronchoscopic biopoy was positive in 64%. Bronchial washing was positive in 38% . Those who did not have bronchoscopy had distant metastasis, mostly to the deep cervical chain, except in one case who had superior vena cava obstruction.
Of the 84 cases, 37 cases or 44% were operable. Eight of these refused the operation and exploration was done in 29 cases. Of these, 14 cases or 17% had a resection. Pathological classifications were discussed. The duration of the survival in reoectable cases were presented, though the duration of the follow up is too short to be of any significance.
